Lode Runner for the Apple II, the original version developed by Douglas Smith and published by Broderbund. The original puzzle-platformer, and favorite game of Tetris-designer Alexey Pajitnov. Unlike the later Arcade and home ports, this original version features 150 levels of pain. source

An excellent blend of Ultima’s exploration mode with a Wizardry-style combat mode [with hints of Ultima’s]… in a post-apocalyptic setting. When Interplay was working on Fallout nearly a decade later, this is the game they were channeling. source
